Residential Habilitation Services
Residential Habilitation (Res-Hab) is a Medicaid-reimbursed service, which is provided in an individual's home by a Residential Habilitation Care Provider and an agency Program Coordinator. This service includes setting goals specific to the individual's desire to move towards increased self-sufficiency while assuring there are adequate supports available to maintain the individual's health and safety.
Program Coordination can include (but is not limited to):
- Writing the consumer's annual Implementation Plan (IP) and other required reports.
- Encouraging reasonable independence and community participation through goals and objectives included in the IP.
- Monitoring the consumer's progress in skills development.
- Meeting with the consumer and the care provider in the home at least quarterly to review documentation and satisfaction with services.
- Providing oversight, regular training, and quality assurance for the Residential Habilitation provider.
- Advocacy supporting the consumer.
Who is Eligible?
Adults (18 years and older) who have been determined eligible for the Developmental Disability (DD) Waiver by the Department of Health and Welfare. To determine eligibility, contact the Developmental Disabilities Care Manager through the Department of Health and Welfare at (208) 769–1567.
Where are Services Available
Residential Habilitation/Program Coordination is available in the five northern Idaho counties of Bonner, Boundary, Kootenai, Latah and Shoshone. A program coordinator can meet with you in your home, the community, or at TESH.
