Plan Development
Plan Development is also a Medicaid-reimbursed service available to adults with a developmental disability. A plan developer is responsible for writing a consumer's Individual Support Plan (ISP) and submitting it to the Independent Assessor with necessary documentation. A plan developer will also attend a consumer's annual budget meeting and submit addendums if the individual wants to add or change services.
Who is Eligible?
To determine eligibility, contact the Developmental Disabilities Care Manager through the Department of Health & Welfare at (208) 769–1567. Targeted Service Coordination (TSC) and Plan Development is available to eligible adults age 18 years and older.
Where are Services Available?
TSC and Plan Development are available in the five northern Idaho counties of Bonner, Boundary, Kootenai, Latah, and Shoshone. A service coordinator is available to meet with you in your home, in the community, or at TESH.
